More detail about the role: The Stroke Practitioner service in North Devon are a small but expanding team. They are involved at every stage of the patient journey within the Stroke Service to support the patient and facilitate seamless transition of care from the Emergency Department to the Stroke Unit, and through liaison with Community teams at point of discharge. They work closely with all relevant Departments, and colleagues across the Multidisciplinary team to achieve coordinated care that meets national and local guidelines.

The team provides a point of contact and expertise in the stroke speciality to staff across the organisation as well as patients and their families/carers. The Stroke Practitioner Service receives direct referrals within the speciality and is able to provide expert assessment and evaluation of patients needs. A key aspect of the post is to provide education, training and support to staff, patients and their families. The team works closely and collaboratively with the Stroke leads and teams in the Emergency Department, the Nursing, Therapy and Medical teams within the Stroke service to develop pathways of care in North Devon and to ensure high quality interventions and care which meet the demands of the new national guidelines and SSNAP.

Innovation is central to the Stroke Team at the Royal Devon University Healthcare NHS Foundation Trust, demonstrated by the implementation of several new initiatives and the development of non-traditional roles in recent years including in ACP in Stroke Services and an AHP consultant within the Neuro rehab Service.
