The focus of the role is to work alongside the multidisciplinary team to support the needs and wellbeing of a person living with dementia during an acute hospital admission. This may include using creative skills to engage with the person (such as arts and crafts, music or reminiscence) supporting with activities of daily living and engaging with the persons family, friends and carers to ensure the clinical team can continue to support their needs.

This is a rotational post, which provides the opportunity to work in a variety of clinical settings across Leicester’s Hospitals, including Older People’s Medicine and Orthopaedic wards and the Emergency Department. This may include working a combination of long days and shorter shifts between the hours of 8.30am - 8pm across seven days.

To facilitate appropriate activities for a caseload of patients on specified wards within UHL who have been identified as having confirmed or suspected Dementia or experiencing a delirium. This will include working with individual patients and organising group activities.

To provide practice based training and support to staff and volunteers in delivering meaningful activities to patients

To work with Multi Disciplinary Teams, and carers to ensure that activity provision meets the identified needs of the patient.

To feedback observations of patients’ involvement in activities to members of the multidisciplinary team. This will be written and verbal.

To liaise with carers, outside agencies, volunteers, hospitals and community resources etc as appropriate to ward activity provision maintaining a professional manner at all times

To support and encourage ward staff to organise appropriate activities in addition to those provided by the meaningful activities staff.

To undertake administrative and organisational tasks as delegated.

To link in with local induction for volunteers

To be responsible for ensuring a safe working environment, conducive to providing meaningful activities

To order and document resources required to carry out meaningful activities and ensure immediate access is available for ward staff and volunteers where necessary

To support fundraising activities

Evaluate and process allocated referrals from different ward areas in order to ensure that a person living with dementia and the clinical team receives support in a timely and meaningful way.

Use a person centred approach to support a person living with dementia to have access to activities which enhance their wellbeing

Ensure good communication with nursing staff prior to all activities to ensure any risks are identified and to raise any concerns noted appropriately

Ensure that duties are not undertaken if they are beyond the competence of the post holder’s level of experience or qualification

To be able to utilise feedback mechanisms and create evidence to promote the service

To assist in monitoring and evaluating the service as directed to provide feedback and statistical data to inform future development of the service

Support members of the multidisciplinary team and volunteers to engage with people living with dementia

Ensure that activities are provided in a culturally sensitive manner

Complete accurate documentation which highlights the engagement you have had with a person living with dementia are providing both written feedback in addition to verbal and to the Multidisciplinary Team

To have an awareness of and promote all initiatives to support the wellbeing of older people and people living with dementia across Leicester’s Hospitals

Ensure that allocated equipment and materials are correctly stored, maintained an
