The Care Navigator role provides the first point of contact for all patients and acts as a focal point of communication between patients, doctors and other medical staff.You will support our patients to ensure that they are signposted to the correct service, offering self-help advice or booking with an appropriate clinician in the practice's multi-disciplinary team. The ideal candidate should have excellent customer service skills a polite telephone manner, great attention to detail, with excellent communication skills, computer literate and be able to multitask in a fast-paced environment. Core Duties & Responsibilities: Support the Clinical Triage Team via Patchs To work collaboratively with other teams and services to maintain an effective and efficient service To offer appropriate support and guidance to patients and their families/carers To have excellent IT skills, to include Microsoft Office, and digital solutions Maintain and monitor the practice appointment system. Process personal, telephone and e-requests for appointments Answer phones in a professional manner to ensure efficient medical care is delivered to patients Signpost patients to the correct service, initiating contact with and responding to, requests from patients, team members and external agencies Accurate data entry of patient information Input data into the patients healthcare records as necessary Manage all queries as necessary in an efficient manner Monitor and maintain the reception area and notice boards Support all clinical staff with general tasks as requested Maintain an orderly work area and cleanliness of the front desk area Perform other office duties such as photocopying, filing, typing, scanning, etc Ensure all office equipment is always maintained and functioning properly Perform other office duties as assigned by management or supervisor
