**Successful applicants will be invited to attend an Assessment Centre at The Town Hall, New Road, Callington on 27th March 2023. This will be a morning or afternoon to be confirmed.**

We have an exciting opportunity has arisen for an enthusiastic, innovative, flexible and well-motivated Senior Community support worker to join Caradon Homefirst team.

The Homefirst team is part of our Integrated team working in Liskeard, Looe, Saltash, Torpoint, Launceston and Callington. supporting the patients in our Caradon Primary Care Network.

It will be an expectation of this post that on occasions, the person works flexibly across other areas of care.

Being able to drive and having access to a vehicle are essential for this post. Mileage expenses will be paid

You will be well supported by your local team who will offer regular clinical supervision. Continuing professional development will be encouraged and funded.

The service operates 8-8 over 7 days per week. We offer a flexible approach to rostering to accommodate personal working patterns

**Main duties of the job**:
The post holder will provide a versatile and responsive service to ensure that opportunities for patients reablement and rehabilitation are optimised within the community setting. The post holder will be part of an Integrated Care Pathway for patients that involved a range of services to prevent hospital admission and support discharge from hospital.

The post holder will undertake specific therapeutic support work to aid reablement and rehabilitation of patients, planning and implementing interventions and reporting back on progress and any issues to the clinician.
