Accountable to: Clinical Lead GP on behalf of Primary Care Network / Other management.

Base location: The post holder(s) will be required to work across all practice geography in Workington PCN

**Job Summary**

The Health and Wellbeing Coach will be responsible for delivering group and one to one sessions on healthy eating, physical activity, chronic disease management and health and well-being coaching to meet with personalised care plans.

The Health Coach will play a critical role in engaging patients, using health coaching techniques, and supportive interventions to support them to take an active role in their health and wellbeing for future wellness and health improvement goals.

The post holder will work autonomously and support patients of all ages and their carers to manage their physical and mental health and wellbeing, build community resilience and make informed decisions and choices when their health changes. Working alongside the GP Practice staff, including the chronic disease management team will form part of this role. Knowledge of Qof and health care requirements is essential to give the best to our patients.

The individual will deliver a universal and proactive offer of support to people with physical health conditions, chronic long term health conditions and mental health conditions to build knowledge, skills and confidence to live well with their health condition, or to feel better and aid better wellbeing for a healthy future.

Workington Primary Care Network are looking for Health and Wellbeing coach to work as part of our multi-disciplinary teams and community services within our personalised care team. They will provide a comprehensive service to patients targeting the following local population health priorities:

- Weight Loss and Obesity
- Wellbeing and Motivation
- Chronic Disease Support.
- Mental Health

This is a role with the primary care network and we expect the post holder to contribute to the development of the role and the services locally.

We are looking for a motivated individual to join the team who are able to work independently to manage their own workload, and are flexible to respond to the developing nature of the role and the needs and priorities of individuals requiring support.

You will be expected to work collaboratively with the GP practice staff, and a wide range of partner organisations, professionals and volunteers. The attendance and contribution to multidisciplinary meetings including safeguarding and other patient meetings will be a requirement of the role either via teams or face to face.

Hours of work should reflect general practice hours 37.5 hours per week within 8.30am to 6.30pm. Occasional evening and weekend work may be required to promote the service and or local organisations. Flexibility is essential for this role.

**Main duties & responsibilities**

The post holder will work closely with the other members of the primary care team and complex care teams in the management/decision making about care and service provision for individual patients of low to medium complexity who will usually have one or more condition.
- Manage a caseload in accordance with the health and wellbeing needs of our population, through taking an approach that is non-judgemental and based on strong communication and negotiation skills.
- Provide personalised care support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently.
- Coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.
- Providing interventions such as self-management education and peer support.
- Supporting patients to establish and attain goals that are important to the patient.
- Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.
- Working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing.
- Increasing patient motivation to self-manage and adopt healthy behaviours and choices.
- Utilising health coaching skills to support people with lower levels of activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
