We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in West London. We are looking for a band 6 Physiotherapist within discharge to assess and Urgent community response


As a member of the Rapid Access Team, you will be expected to be a competent Physiotherapist who is able to adapt to any of the 3 pathways: VW (Virtual Ward), UCR (Urgent Community Response) and D2A (Pathway 1 Discharge to Assess).

You will be part of a fast paced, ever evolving community MDT, where your proven ability to make high level clinical decisions will aid patient recovery. This will include having a direct link to the patient’s discharging medical and therapy team for liaisonand support where necessary, and with the patients GP or other health care professional to prevent hospital re-admission.

As a Band 6 Physiotherapist, you will be expected to carryout complex risk assessments and make high impact clinical decisions. You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care. As aphysiotherapist you will be expected to be competent in taking observations and completing NEWS2 scores on patient’s, interpreting these scores and identifying and escalating when a deteriorating patient. It is essential you will use evidence-based principles/currentbest practise, along with specialist clinical skills, to assess, plan, implement and evaluate interventions of a varying degree of complexity.

**Position Requirements**
- HCPC Registered
- 3 month (minimum) locum assignment to start as soon as possible
- Full time hours (Monday - Friday)
- Minimum of two years postgraduate experience
- Previous clinical experience performing a similar role within a Community setting
