Health Youth Justice Teamis a part of NHFT, a county wide CQC accredited Outstanding trust offering health services to children, young people and families in Northamptonshire.

An exciting opportunity has become available for an assistant Practitioner to join the Health Youth Justice Team. The Health Youth Justice Team supports young people’s wellbeing, physical health and speech, language and communication needs. We work with the Northamptonshire Youth Offending Service, Police and other NHFT services.

The role will require the applicant to have experience in working with young people with complex needs. This role will also include working in partnership with other statutory agencies, particularly Northamptonshire Youth Offending Service (YOS) and the Police

If you have experience of working with young people and are passionate about supporting the health needs of young people accessing youth justice system we would like to hear from you.
- To work in Health Youth Justice Team alongside Youth Offending Service according to each service’s practices, ethos and responsibilities.
- To complete screening assessments to identify interventions that would be most appropriate for individuals.
- To provide direct support and advice to young people and their families, and professionals.
- To arrange and offer intervention plans with young people and their families.
- To assist senior staff in undertaking projects and see them through to completion within agreed timescale.
- To monitor outcomes using accepted outcome measures, and ensure these outcomes are fed back to children/young people and their families/carers appropriately.
- To represent the service at meetings with partner agencies, with other local statutory and voluntary organisations and within the local community

NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
