Advanced Practitioner

The focus of the Advanced Practitioner is on ensuring that practice standards are maintained and that assessments are strengths-based and person-centred, whilst ensuring that interventions represent a proportionate response to presenting issues. The Advanced Practitioner will work closely with the Practice Manager and focus on quality assurance for practice related issues within the teams through supervision and informal support for staff. The Advanced Practitioner will give guidance to staff to support their decision-making and to help them tackle challenging conversations and conflict. The Advanced Practitioner would not be expected to deal with budgetary, staffing or team performance issues but is likely to hold a small, complex, caseload.

The Short-Term Services team works to Home First principles, and in close partnership with colleagues in acute and community health services, social care, and the voluntary sector. The local health and social care economy are working to embed and improve Home First working and with continued service development it is an exciting time to join the team. The team is community based with in-reach into hospital and community bed settings. The team is innovative, friendly, and supportive, and there are good opportunities for training and professional development within the job role.

What we are looking for:

- Enthusiastic experienced Social Workers who have a good understanding of adult safeguarding, mental capacity and Best Interest decisions making.
- An understanding of practice standards to ensure they are maintained and strength-based and person-centred assessments.
- It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
- Experience of supervision and quality assurance would be beneficial but not essential.

The post holder will be based at Delamere House in Crewe, but hybrid working is available and includes working from home, there may be a necessity to travel to other bases. Cheshire East operates a 'flexible and mobile' working policy.
