**Job Summary**

**The Role**

The Recovery Practitioner Homelessness Specialist role focusses on supporting Rough Sleepers, as well as individuals housed by London Borough of Merton’s Homeless Person’s Unit, delivering psychosocial treatment, and supporting those individuals to achieve recovery and through this mechanism maintain their accommodation. This role will work closely with multiple partner agencies within the council and other third sector organisations.

The Recovery Practitioner Homelessness Specialist will also conduct outreach to support those individuals who may struggle to attend the service, attending satellites across the borough, conducting home visits, and delivering assessments and psychosocial interventions in diverse environments.

**The Service**

Launched in 2018, Via Merton is a fully integrated substance misuse service, providing clinical and psychosocial interventions, operating from a main treatment hub, with additional satellites within the community. The team actively promotes recovery and works with individuals that are directly and indirectly affected by substance misuse, working with the local community to improve the health, wellbeing and functioning of Merton residents, irrespective of circumstance.
