Are you looking to make a difference in primary care? We are looking for a Health and Wellbeing coach (HWBC) to work across North Derbyshire PCN's member practices within Chesterfield and the surrounding areas. HWBCs use coaching skills to help individuals to identify, set and track personalised health and wellbeing goals and will provide support on a 1-1 basis or via group coaching sessions.

**Salary**: £23,000-26,000 per annum

Health and wellbeing coaches help to empower patients to take control of their health and wellbeing with health improvement changes such as weight loss, eating well, increasing physical activity levels, and stopping smoking.

Your role will be to support personal choice whilst also ensuring patients understand the results of his or her own actions and decisions. The post holder will need to communicate effectively with a wide range of multi-disciplinary colleagues both in house and externally, and be able to take the complex needs of individuals and the varying requirements of the post into account in order to prioritise work undertaken. The post holder will actively work to continually develop the service in a patient-centred, evidence-based context.

Key responsibilities
- Managing and prioritising a case load in accordance with the health and wellbeing needs of their population through taking an approach that is non-judgmental, based on strong communication and negotiation shills, while considering the whole person when addressing existing issues. Where required and as appropriate, the Health and Wellbeing Coach will refer people back to other health professionals within the PCN.
- Liaising with multidisciplinary teams to identify patients who would benefit from health coaching.
- Utilising existing IT systems to identify people that would most benefit from health coaching.
- Provide personalised support to individuals, their families and carers to support them to be active participants in their own healthcare, empowering them to manage their own health and wellbeing and live independently.
- Work in partnership with the broader multi-disciplinary team colleagues.
- Work closely in partnership with care coordinators and other practice staff to identify and work alongside people who may need additional support.
- Coaching and motivating patients through multiple sessions to identify their needs, set goals and supporting patients to achieve their personalised heath and care plan objectives.
- Providing interventions such as self-management education and peer support.
- Supporting patients to establish and attain goals that are important to the patient.
- Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.
- Working in partnership with local community services to connect patients to community-based activities to support them to take increased control of their health and wellbeing.
- Increase patient motivation to self-manage and adopt health behaviours
- Work with patients to understand their level of knowledge, skill and confidence when engaging with their health and wellbeing and subsequently supporting them in shared decision-making conversations.
