**Strategy**
Supporting the successful operational implementation of our Care Strategy, incrementally
increasing the number of children and families who access children’s palliative care services across Shropshire, Cheshire and Mid and North Wales.
**Continuity of Care**
Participate in the promotion of a culture where palliative care is delivered in a friendly, welcoming and non-institutional environment, which is safe and fit for the needs of neonates and their families.
Deliver support with empathy and understanding, using a person-centred approach to put the neonate and their family at the heart of everything we do. Actively maintain professional relationships with families to enable honest and open communication by developing a trusting relationship.
