**Job Summary**

As a Social Prescriber you will play a crucial role in the NHS’s commitment to holistic care. You will work alongside GPs and other healthcare professionals to identify individuals who would benefit from non-medical support in their communities. Your mission is to connect them with services and activities that address their social, emotional and practical needs, ultimately improving their overall health and wellbeing.

**Key Duties and Responsibilities**

Build strong relationships: Develop rapport with individuals referred to you, actively listening to their needs, aspirations and challenges

Create personalized care plans: Collaboratively develop plans with individuals, focusing on non-clinical interventions like social groups, volunteering opportunities, exercise programs, or skills development

Navigate the community network: Leverage your knowledge of local resources to connect individuals with relevant services, including employment support, financial advice, befriending schemes, or cultural activities

Empower individuals and communities: Encourage people to take control of their health and build their own networks of support. Facilitate the creation of new community groups or activities to address unmet needs

Collaborate effectively: Work closely with GPs, healthcare professionals, and community organizations to ensure seamless care and resource sharing

Monitor and evaluate impact: Track progress of individuals and contribute to data collection to demonstrate the effectiveness of social prescribing interventions.

**Experience**

Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)

Experience of supporting people, their families and carers in a related role

Experience of partnership/collaborative working and of building relationships across a variety of organisations

Ability to maintain effective working relationships and to promote collaborative practice with all colleagues

Ability to work flexibly and enthusiastically within a team or on own initiative

**Skills and Knowledge**

Broad knowledge and understanding of how the NHS works, including primary care and MDT working

Knowledge of the personalised care approach. Utilises the evidence base for social prescribing interventions and activities.

Understanding of, and commitment to, equality, diversity and inclusion.

Excellent written and verbal communication skills

Organisation skills with the ability to constructively challenge the view and practices of managers and clinicians

Effective time management (planning and organising). Be flexible and able to manage sudden and unexpected demands

To be a strategic thinker and planner with the ability to consider and act upon complex issues

Demonstrate personal accountability, emotional resilience and work well under pressure

Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences, including patients

**Salary**: £12.00-£13.00 per hour
