The Discharge to Assess trackers role supports the safe discharge of patients with complex needs

The role is pivotal to safe, effective discharges of our patients

It is essential that the post holder has the confidence and ability to communicate sensitive information to patients, carers all levels of staff.

The Discharge to Assess Trackers work within the D2a Units as part of the MDT (Physiotherapist, Occupational Therapist, Discharge Co-ordinators and Social Workers). They are the main point of liaison regarding discharges.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require a Standard DBS check at a cost of £26.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

To provide information to Patients regarding their discharge

To provide the therapy team with updated information

To ensure all referrals are completed in a timely manner

To liaise with all members of the MDT and out of area teams

To support the Discharge Coordinator in maintaining good patient flow throughout the unit
