The Young Adult Transition and Learning Disability Team have an exciting opportunity for two re-ablement assessors to join our dynamic and dedicated team. The role will involve working closely with our partners, young people, adults and their carers who may be eligible for adult social care under the Care Act 2014. The team currently support people from transition into adult services through to older age who may have a range of support needs and disabilities.

You will need good administrative skills and a proven experience of developing, maintaining and inputting information on electronic based systems. You will be able to demonstrate analytical skills and effective decision-making within appropriate timescales; demonstrate a person-centered approach and be able to act in a tactful and sensitive manner with people who access services.

Reablement is a strengths-based, person-centered approach that promotes and maximizes independence and wellbeing. Under the umbrella of Safeguarding you will undertake reablement assessments with a view to implement support, monitor and review to enable adults and young people to develop/regain/re-train skills and levels of independence to ensure positive change using user-defined goals.

You should have recent relevant experience of working in a Social Care setting. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, ideally including knowledge of current developments and service pathways.

The successful applicant will receive regular supervision and an Annual Review, be well supported and have opportunities in respect of their career development, including experiencing a variety of roles within adult social care.
