ROLE PURPOSE Wellbeing Coordinators provide a holistic and person centred point of contact for patients and those close to them. You will facilitate an excellent patient experience across our Wellbeing and Community services, and work with the Wellbeing Centre & Outreach Lead in the coordination and delivery of Wellbeing Centre activities both onsite and in our communities. You will be the first contact for new people referred to the hospice, and coordinating a range of internal and external services based on what is most important to them. You will provide people with information, giving them time to focus on what matters to them.

You will support communication of the persons needs within the wider multi professional team. There will be some cross cover for the Clinical Administrators and Advice Line assistants when required. Main duties/responsibilities: Make initial contact with people referred to the service, gathering information of what is important to them Work within the multidisciplinary team to support the coordination and sourcing of services in a timely, professional and responsive manner To liaise with internal and external referrers to facilitate the persons need being met - this may include a visit to the patients home to talk through any concerns, completion of referrals, help with paperwork or forms. If required, attend relevant hospice multidisciplinary meetings to discuss any relevant issues to ensure safe, well planned and co-ordinated care.

To provide a consistent and trusted point of contact for people and those close to them Record accurately all relevant information and escalate to the multi-disciplinary team as required. To develop and maintain updated knowledge of a wide range of external local services. Take part in and support the wider function of the Clinical Administration team function as business needs dictate To research, identify, signpost and coordinate access for people to groups, activities at the Wellbeing Centre or elsewhere. Actively support Wellbeing Services to provide holistic patient and family care Work with the Wellbeing Centre & Outreach Lead in the coordination and delivery of services, including the orientation of new patients to the Wellbeing Centre, organising hospice tours, outpatient clinics, groups, activities and more Quality and Service Improvement To participate in audit projects and research.

To participate in the hospice Governance procedures. To adhere to hospice and professional policies and procedures. To promote and engage in new ways of working, such as the use of technology Training and Development To ensure personal compliance with hospice compulsory training requirements. To act as a role model for employees and volunteers, participating in personal development initiatives as appropriate and living the hospice values.

To assist in orientation of new colleagues, volunteers and visitors. Through the performance development process, to plan your continuing professional and self-development, attending in-house study days and courses as appropriate. To participate in clinical supervision, debriefing and reflective practice sessions This is an outline job description designed to give an overview of the responsibilities of the role. We expect the job holder will work flexibly, responding to organisational need and changes as they occur.

You will also contribute to the wider corporate and organisation needs of St Catherine's such as supporting our fundraising efforts.
