Are you an experienced Advanced Clinical Practitioner (ACP)with community health service experience?

We can provide you with the support and the tools for the role and offer you the chance to develop and diversify your career. As the Lead ACP, you will be responsible for supporting thesenior leadership teamwith clinical effectiveness activities, clinical governance, CQC readiness and service improvements.

The community lead for advanced clinical practice will be responsible for the community services development and leadership of Advanced Clinical practitioners (ACP). They will support the ICC Divisional Director of nursing in developing, leading and implementing a multi professional, organisational advanced practice strategy. They will provide highly specialised, expert advice, education, and clinical expertise.

The post holder will also be required to practice for a minimum of 20% as an ACP and as such will be required to deliver expert advanced level of clinical practice within their specific field. Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients which may include the prescribing of medicines, or referring patients to an appropriate specialist.

- On behalf of the Divisional Director of Nursing takes lead responsibility for the development of a community service advanced clinical practice strategy that reflects local, regional and national priorities, supported by the Director of Nursing, Midwifery and AHP Research.
- To take overall lead responsibility in providing effective professional leadership and governance to the ACPs within community services.
- Develop and maintain effective working relationships with local HEI providers whilst considering alternative methods of learning
