Tuag Adref is looking to add to their team a confident member to be able to expand on the service provided to patients in their own homes to rehabilitate and promote independence. There is an element of palliative care within the service.

There is close working with numerous agencies within the community to be able to provide care closer to home and reduce the need for hospital stays or delayed discharges.

The successful applicant will be in a role that will support the HCA working within the community as well as supporting the coordinator of the service. There will be a need to review referrals and accept onto the caseload as well as arranging with the MDT to create end plans for the patients. There is an element of data collection that does require IT skills to input and understand their importance.

Ability to travel between sites in a timely manner.
