**ST HELENS KEYWORKER POST**

Do you want to be part of shaping an innovative Keyworker support service for children, young people and young adults in St Helens? Can you support and achieve transformational change for children, young people /young adults with a learning disability, autism or both?

Each young person/young adult referred to the service will have an named assigned Keyworker. The team will provide expert planning, support navigation and advocacy to the young person/young adult and their family at a level appropriate to the person's needs. A “step up and step down” process will ensure young people/young adults receive when the right level of support when they they need it and can re access their key worker as required.
- Are you passionate about supporting children and young people/young adults to navigate services and support? Do you have experience of working with young people/young adults with autism, learning disabilities and mental health, experience of working in social care, health or education setting, experience of multi - agency working / partnership working, experience of evidence of report writing and case management, if you do, these exciting new opportunities may be for you._
