**JOB SUMMARY**

To provide person centred support to adults with acquired brain injuries and their families in the community. To work alongside colleagues and volunteers in the delivery of support. To ensure activities encourage independence and facilitate an improvement in mental and physical wellbeing whilst supporting individual interests and future needs.

**MAIN RESPONSIBILITIES**
- To complete assessment forms, regular reviews and feedback of the community wellbeing service
- To act as a Wellbeing Support Worker to adults with acquired brain injuries in the community and / or in their own home
- To facilitate an agreed programme of activities with the client
- To complete all administrative tasks associated with individual support, at the office or clients’ home
- To ensure data is input on centre systems appropriately as and when required
- To deliver a high-quality user led service
- To complete evaluation forms with clients
- To assist volunteers working within the community service
- To meet with The Charity Manager regularly to discuss the service and to provide detailed reports as and when required

**Service delivery**
- To ensure activities enhance cognitive and social wellbeing and are provided in such a way as to promote independence, as far as possible
- To assist in the monitoring and evaluation of services alongside the Charity Manager
- To ensure that clients play an active role in planning and evaluating activities and opportunities

**Health & Safety**
- To ensure appropriate risk assessments are completed and safety measures are adhered to at all times
- To respond to any medical emergency that may arise
- To ensure that all accidents / incidents are reported to the Charity Manager and recorded in the accident / incident book
- To inform the Charity Manager immediately of any health and safety issues or concerns

**General**
- To promote a positive image of Brains Matter at all times
- To support the aims and objectives of Brains Matter and adhere to policies and procedures and quality standards contributing to their development as required
- To liaise and meet with the Charity Manager on a regular basis
- To work within budgetary constraints
- To support the Charity Manager, staff colleagues and volunteers in ensuring high standards and continuity of service delivery
- To maintain confidentiality at all times
- To attend meetings and report to the Board of Trustees as requested
- To attend appropriate training courses/seminars as required or requested
- To attend evening or weekend meetings or events, including fundraising and marketing, in line with the role’s responsibilities
- To undertake alternative duties as and when required to cover any staff absences or to ensure high standards and continuity of service delivery

**PERSON SPECIFICATION**

**EDUCATION & QUALIFICATIONS**
- **Essential**_

GCSE/O-levels in English and Mathematics or equivalent

A background or qualifications in health and/or social care

Basic skills in Microsoft Office
- **Desirable**_

NVQ Level 2/3 in Social Care
- Mandatory and specific training on brain injury issues will be offered_
