**About the Role**:
Hilton Primary Care Network has an exciting opportunity for a first contact practitioner to join our Bedfordshire-based PCN, working across our three practices: Houghton Close Surgery in Ampthill, Greensands in Ampthill and Dr Hughes and Partners, Ampthill. This role is offered on a part-time basis, working three days per week.

Whilst undertaking comprehensive assessment and diagnosis of patients with a wide variety of physical and emotional needs, you will formulate, deliver, review and progress individual programmes and ensure effective treatment. Other regular tasks will involve supporting the GP partners and healthcare professionals across our practices.

You will:

- be a driven and focused therapist, who will be joining a new/developing service.
- be a specialist in MSK, who is passionate about the consistent delivery of quality clinical management of musculoskeletal conditions.
- work as an autonomous practitioner and will be supported by the wider clinical teams.
- have excellent clinical and leadership skills, be proactive and self-motivated
- have a passion for maintaining high standards of care and developing services with the patient at the centre of your service delivery.
- ideally have strong experience in either a community or primary care setting and be confident in analysing and presenting information to health professionals.

Some flexibility is required as you will sometimes need to work additional or alternative hours to cover for colleagues.

Serving approximately 33,000 patients, across Ampthill and Barton-Le-Clay, Hillton PCN strives to develop collaborative working across the three member practices, aiming to support and strengthen the delivery of primary care services in line with the NHS Long-Term Plan.

We aim to achieve high standards of patient-centred care as identified for the specific needs of our population, whilst working to support a resilient and appropriately trained workforce. We operate SystmOne clinical system with a good history of QOF achievement and a CQC rating of Good across all domains. We currently have a paramedic, three clinical pharmacists, a care coordinator, a social prescriber, two first contact physiotherapists and a primary care network manager. We are also recruiting for further roles to support the future of the network.
