Community Health Care Assistant

We are currently recruiting health care assistants to expand our successful Palliative Care Support Service. You will be part of a dedicated team caring for people with life-limiting conditions in their own homes, committed to delivering high standards of care to patients and their families.

The post holder will act as a member of the community palliative care team providing overnight support to patients in their own home. They will work unsupervised undertaking a broad range of duties relating to the care of patients with end of life/palliative care needs in their homes.
Work within the policy framework of the hospice in its provision of specialist palliative and supportive care by ensuring that the clinical aims and objectives of the organisation are met.
The Hospice operates a KSF-based workforce development programme, which incorporates a detailed method of Personal Development Review (PDR). The competence and performance of our staff is important to us; you may be asked to attend training and development some of which may be mandatory.
North London Hospice is a registered charity, that has been caring for local people since 1984. The Community teams cover three London boroughs Barnet, Enfield, and Haringey

- Provide direct patient care where it has been identified this will support patients in their preferred place of care/ preferred place of death and prevent unwanted /unnecessary admissions to hospital. Working with the PAT, and DN service to manage ongoing care
- Under the supervision of a CNS, monitor and review the effectiveness of interventions with patients to ensure care is patient centred.
- Deliver high quality care as required to patients as detailed in care plans, reporting any changes in condition to the CNS team.
- Respect people’s dignity, wishes and beliefs and involve them in shared decision making, obtaining their consent.
- Act as an advocate for the patient when necessary.
- Provide basic support, information and advice to bereaved family members in liaison with Social Worker and Bereavement Support manager
