Seven Resourcing are looking for a number of temporary Link Workers to work with the Accommodation and Placement Support Service in Billingham. This position is initially for a period of 3 months and is paying £13.44 (PAYE) or £16.64 (umbrella) per hour.

**The role**:
Provide a keeping in touch service during times of significant distress and crisis to children, young people and families/carers including out of hours and on a weekend on a rota basis.
Keeping in touch, supporting and linking in with families, carers and children and young people in fragile placements, providing assistance to identify needs.
Contacting and visiting children, young people and families/carers in the community and at home to respond and provide action to identified needs.
Providing a signposting role and during times of significant distress for families and children taking a proactive role as the link to other services and or professionals to ensure support is accessible. This may include accompany children and young people, families and carers to appointments or other, as identified and needed in order clients are helped to navigate through services.
Checking, verifying and updating personal details as appropriate on case management systems.
Work flexibly across Children’s Services and with other agencies, partners and stakeholders to meet the needs of children, young people and families.
To assist on specific projects and take on responsibility for other areas of work when required.

**Key Responsibilities**:

- Working to a schedule of regular contact or responding to a crisis situation with children, young people families and carers as outlined in a support/care plan.
- Exploration and establish with children, young people, their families, carers or support network, and other agencies, their current situation, identifying further support needs from partners.
- Link in and keep regular contact with families and young people via telephone contact, home visits and any other effective means.
- Promoting other services to children, young people, families and carers and encourage access, help to clients to navigate through the system and link the client to services and take reasonable steps to ensure that the young person and families makes use of such services.
- Maintaining client destination information on to the case management system.

**Applicant Requirements**:

- **Qualifications**: Recognised Level 3 qualification in an appropriate discipline e.g. health and social care, education.
- **Experience**: 2+ years working within a similar role.
- **Compliance**: Current DBS check and references covering the last 3 years.
- **Travel**: Clean UK driving license with the ability to commute to and from work independently.
- **Hours**: 37.5 hours p/w.
