Camden Carelink & Phlebotomy Service provides community reablement and case management to service users living in their home/usual residence community to promote independence, and prevent unnecessary hospital admission, and reduce the length of stay forthose admitted to hospital. It’s clinical team is comprised of Occupational therapists, a Nurse Educator, community health support workers and phlebotomists. Visits may be made on foot, by public transport, bicycle, motorcycle or pool car.

To screen all referrals received into the service and manage a caseload of clients with simple to complex needs with supervision and guidance as necessary from senior staff.

To successfully conduct, monitor, evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated.

Develop and conduct occupational therapy intervention with clients.

To work autonomously in a community setting and undertake visits or cover for Coordinator staff where necessary.

To update and monitor the safe staffing/acuity and dependency assessments of clients To be physically able to install specialist occupational therapy equipment in a client’s home.

To ensure client and service records are recorded and maintained on IT databases and that client documentation is managed according to trust and local policies To prescribe, delegate and monitor the work of Community Health Support Workers.

To educate and teach the relevant health workers to participate in on-going reablement and care programmes for clients, as appropriate and provide supervision support to team.

**Position Requirements**
- HCPC Registered
- Minimum of two years postgraduate experience
- Previous clinical experience performing a similar role within a Community setting
