**Counselling Services**: Provide individual and group counselling sessions to patients and their families, addressing emotional, psychological, and spiritual concerns related to illness, loss, and end-of-life care. Offer a safe and confidential space for clients to explore their feelings, thoughts, and experiences, using evidence-based therapeutic techniques. Provide age-appropriate counselling and support services adapting counselling techniques and approaches to meet the unique needs of each client. Offer support in person, over the phone, or virtually via video call, ensuring accessibility and flexibility in service delivery.
Requirement to access internal/external counselling supervision on a bi-monthly basis. Training & supervision of student counsellors.Bereavement Support: Offer compassionate bereavement support to individuals and families who have more complex needs and are experienced the loss of a loved one, providing emotional guidance, coping strategies, and resources to navigate the grieving process. Facilitate bereavement support groups and cafes to foster peer support and connection among bereaved individuals. To assist in the recruitment of new bereavement support volunteers.
Preparing and implementing their training. To allocate clients requiring bereavement support to volunteers matching with relevant skills and qualities To quality assure bereavement support volunteer record keeping and provide appropriate supervision and development opportunities to maintain their knowledge.Collaborative Care Coordination: Collaborate closely with the multidisciplinary team, including doctors, nurses, social workers, chaplains, volunteers and therapists, to ensure coordinated and holistic care for patients and their families. Participate in team meetings, case conferences, and care planning sessions to contribute insights and recommendations for patient care.Documentation and Reporting: Maintain accurate, contemporaneous, and confidential records of counselling sessions, assessments, and client progress notes in accordance with professional and regulatory standards. Prepare reports and updates on counselling activities, outcomes, and trends for review by the Head of Patient & Family Support and senior leadership.
