Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We are looking for an innovative, passionate, and professional individual with excellent interpersonal and organisational skills to join the Communication Support Service based in West Hertfordshire.

Position: S1062 Stroke Support Coordinator

Location: Homebased, West Hertfordshire however, frequent travel will be required as part of this role (May include team meetings or other work related meetings)

Hours: 35 hours per week

**Salary**: Circa £25,500.00 per annum

Contract: This is a fixed term until 07 August 2024. Services are contracted and there is currently funding for this contract until March 2025.

The Role

The service aims to identify and address the needs of stroke survivors and carers whose lives are affected by aphasia, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

Reporting to the Service Delivery Coach in the East of England, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.

Key responsibilities will include:
*Providing communication support to stroke survivors and carers to develop appropriate communication strategies and practices

*Providing personalised information, advice and support.

*Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.

*Working with other health and social care professionals across the stroke pathway to ensure high quality support.

*To run an effective service for stroke survivors and carers, including face to face, digital and online resources, ensuring that confidential and accurate records are kept on our CRM data base

About You

You will have experience in:
*Experience/ background in a caring profession, ideally supporting people with disabilities.

*Excellent IT skills and an ability to maintain accurate records.

*An affinity with the values of the Stroke Association.

*A flexible approach and an ability to effectively manage a caseload.

To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
