**Job Title: Care Coordinator**

**Location: Northwich, Cheshire**

**Salary: £22,500 - £25,500 p.a. + benefits**

For this role we are looking for an individual who has great communication skills and wants to be part of a fantastic Team who strive to deliver the highest standards of care to their customers who live in their own homes. This is an exciting opportunity to someone who has previous experience working as a Senior Carer or someone who is already an experienced Care Coordinator.

Working for this a Quality driven and highly reputable domiciliary care provider, you will be responsible for ensuring your customers and carers receive excellent quality and professional care.

Previous experience of working in a homecare/community-based care environment
An awareness of how to use a care rota systems and experience of scheduling care calls
Be prepared to take part in the Team based On-Call rota
The ability to work both independently and as part of a Team
Great written and verbal communication skills
To be able to think fast on your feet and make quick decisions

**As Care Coordinator you will be required to: -**

Manage staff rotas efficiently and in advance
Coordinate and allocate shifts in accordance will Carer availability and mileage
Match the right carer to the right client
Maintaining all office policies, procedures, and in-house systems
Carry out some field based activities; assessing new customers, carrying out supervisions with new Carers
Answering and monitoring incoming telephone calls
Be able to carry out audits and quality monitoring checks where required
Assist the Registered Manager with day to day operations
Respond and resolve customer queries
Work closely with the recruitment team to ensure adequate carer capacity
Develop relationships with carers and clients

**Job Types**: Full-time, Permanent

**Salary**: £22,500.00-£25,500.00 per year
