Therapy assistant or band 5 physiotherapist depending on experience.

Role support reablement (hospital discharge), community physiotherapy and falls pathways.

Therapy assistant role as Band 4 to complete initial reviews and make safe. Basix assessment for mobility equipment, progress and deliver therapy programs under the guidance as required of the therapist.
