**Partnering Health Group **has an exciting opportunity for **Advanced Clinical Practitioners to join our team.**

This role could be an ideal opportunity to join PHL on a permanent part time basis. We have evening and weekend hours available and offer home working, depending on circumstances and experience.

As an **Advanced Clinical Practitioner**, working independently via remote telephone triage, you will be **_accountable _**for delivering safe, clinical decision-making and expert care to a high standard. Acting within your professional boundaries, your duties will include aspects of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; prescribing medication and evaluation of their care.

You will be required to work completely independently in an autonomous role at an advanced level of practice. Advanced practice will include evidence of working within the four pillars, clinical, education, research and leadership.

**Advanced Clinical Practitioners Key Responsibilities**:

- Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions.
- Take history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working in any clinical environment.
- Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care for patients when necessary, such as 2-week wait, urgent and routine referrals to specialities.
- Request investigations and be able to interpret results.
- Prescribe medication as required within your clinical competence.

**Advanced Clinical Practitioners Key Skills and Experience**:

- Registered with the NMC or HCPC
- Non-medical prescriber
- Completion of MSc Advanced Practice Pathway or equivalent.
- Minor illness and minor injury qualification.
- Minimum 3 years post registration experience
- 3 years of experience working in a primary care GP practice.
- 3 years’ experience in an urgent care setting such as an urgent treatment Centre or Emergency Department.
- Experience of working in Primary Care
- Demonstrates knowledge of patient group directions and associated policy
- Experience of working in a multi-disciplinary team including mentoring of junior staff.

Rates on this advert are based on a full-time basis plus enhancements for unsociable hours
