If yes, we are offering a great opportunity to join the team in Derbyshire providing personalised advice, guidance and support to people who are worried about their memory and people living with dementia and their families and carers

This role will require you to build relationships with a range of local contacts. You will network with health and care professionals, marketing Alzheimer’s Society services to help people with a diagnosis to get referred to the service as early as possible. As well as developing networks with partner organisations to promote the aim of empowering individuals living with dementia to make informed choices.

**About you**

We provide information, guidance, and support in people’s homes and within their communities.

We are looking for:

- Working experience of assessing people face to face with an ability to assess their needs and an understanding of the need for client confidentiality.
- Excellent non-judgmental communication skills.
- Strong organisational and time management skills.
- Good IT skills and experience working with databases.
- Have demonstrable experience of supporting people affected by dementia or similar.
- An empathetic approach with a commitment to equal opportunity.
- The ability to travel independently across the area.
