As a Nurse Assessor you will work closely with the Area Director and Hospital Liaison Team (HLT) to identify and support homes in the completion of pre-admission assessments.On behalf of the Home Manager, you will use your clinical skills and knowledgeto undertake person centred assessments working with the individual and their relatives to fully understand their care needs.Your detailed assessment will enable the Home Manager to determine if the home is suitable, able to provide the level of care neededand what additional equipment may be required.

Forging strong relationships with Home Managers and Clinical Leads will help you understand any barriers the home is facing, the level of acuity they can support, and any other information which may be pertinent to potential admissions. You will appreciatethe urgency to clear any backlog the home may be experiencing and act upon receipt of any new referrals providing the outcome and recommendation to both the home and referrer in a timely manner.This includes ensuring that information gathered from PrimaryCare and other external agencies is pulled together robustly. Having completed the initial assessment you will be responsible for developing the initial short-term care plan.

Reporting to the Area Director you will enjoy working autonomously with the support of the Regional Senior Nurse in relation to complex clinical situations and decisions.

You will be a Registered Nurse with a valid Pin number and will understand your commitment to your profession through the standards of practice set by the NMC. In addition you will have;
- A person-centred approach that is in keeping with our Kind Care Model
- Clinical experience of assessing Dementia, and fragility.
- A good understanding of complex care
- A track record of working in partnership across organisational boundaries
- Worked across NHS and Social Care

**Qualifications & Skills required**

**Essential**
- Registered Nurse with a valid PIN
- Admission/discharge assessment experience in either social or health care setting.
- Good written and verbal communication skills
- Microsoft office conversant/ good knowledge of outlook/word/excel
- Ability to work flexibly
- Full driving licence

**Desirable**
- A good understanding of health & social care integration.
- A flexible approach to work to meet business demands
