We provide a comprehensive range of children’s health services, so that we are able to look after patients from antenatal diagnosis through childhood, into adolescence and on into adult services.

The Diabetes services at Evelina London includes a dedicated multidisciplinary team as advocated by the best practice tariff including three diabetes nurse specialists, psychologist, dieticians and a diabetes coordinator. We offer services to children with type 1&2, monogenic/neonatal, post renal transplant diabetes, children retrieved through PICU and surgical patients from other trusts as well as other causes of diabetes.

We are looking for a dynamic family support worker to assist our patients and families to manage their condition and improve their compliance and long term health.

The post holder will work both in the hospital and in the community.The post holder will be accountable to the Diabetes Clinical Nurse Specialists and work particularly closely with the diabetes nurse specialists, dieticians, youth worker and the Diabetes team psychologist.

Whilst some knowledge of Paediatric Diabetes is desirable for this post, training in paediatric diabetes will be provided in post.

- Develop a caseload to support the needs, abilities and capacities of children, young people and their caregivers/families
- Collaborate with the referral source and the patient/caregiver/family to develop and implement a patient-centered service plan to meet their social, emotional and developmental needs.
- Facilitate access and referral to appropriate services for additional support that may be required
- Signpost to appropriate parenting education courses and materials for parenting, managing conflict, and motivation in partnership with other professionals in the Diabetes team
- Establish and maintain coordinated working relationships with community social services departments, other professionals and service providers involved with children and families. Assist patients and caregivers/families to access and utilise services.
- Participates in staff MDT meetings, virtual clinics

Document service provided and provide written reports in accordance with established policies and procedures.
Organize and undertake home visits to patient/families and carers following referrals from the diabetes team.
Document patients, caregiver/family’s capacity and support them with managing diabetes.
Communicate any findings/concerns or patient’s, caregivers’/family’s questions to the team.
Facilitate access and referral to appropriate services for additional support that may be required
Signpost to appropriate parenting education courses and materials for parenting, managing conflict, and motivation in partnership with other professionals in the Diabetes team
Operate in accordance with the Trusts child protection policies and procedures. Makes appropriate referrals and supports the Trust social work department in statutory child protection procedures where required.
Maintain an awareness of community resources, including health care services, social, economic, recreational, employment and educational services and resources.
