Job description

37 hour per week contract working on a 7 day a fortnight pattern including evenings/weekends and Bank Holidays.

As a Community Assessor you will be an integral member of our Community Reablement Team. You will be responsible for assessing, reviewing and monitoring reablement services that are delivered by the team and if eligible arrange for ongoing long term care. You will work with residents of Reading in their own homes and travel across the Borough to achieve this.

**About the Role**
You will work closely with a team of Intermediate Care Assistants and manage a busy caseload. Keeping the person at the center of all you do you will need to be creative, innovative and gain knowledge of local services to support the individual in an open and diverse manner. Sound knowledge of using IT is essential. The work is diverse and no one day will be the same.

You will need to work in a methodical and well organised manner. You will plan your own working days, liaise with team members and forge strong links with health and social colleagues. With the ability to motivate and support individuals to achieve their goals for independence you will need excellent communication and recording skills.

As a Community Assessor you will seek funding approval and at times present your case to a panel to justify the support plan you have created. Make referrals to other offices such as finance and set up direct payments if required.
