**Job Overview**

Modality Partnership Mid Sussex Division has a part-time/full-time vacancy for a Clinical Practitioner with specialist interest and experience in diabetes. The postholder will be a high performing qualified Clinical Practitioner working within Modality Mid Sussex Division delivering person centred, safe and effective patient care. **Main duties of the job**

The role is an all-rounded, clinical role; the postholder will need to confidently evidence and utilise key skills such as ability to work on own initiative, ability to develop an evidence-based, personalised, integrated health and social care plan in partnership with the individual, carers, relatives and health care professionals, based on a full assessment of medical, nursing and social care needs, and demonstrate ability to act as an autonomous advanced practitioner providing a comprehensive clinical assessment service to patients/carers/families resulting in safe, appropriate clinical decisions. The postholder will work in collaboration with local divisions (including Divisional Boards) and the National Modality Business Support Team to deliver innovative ways of working to meet the strategic aims of the organisation.

**Qualifications and Experience**:
We require high performing team members to join our team with:
Registered first level general nurse.

Recognised degree qualification in autonomous practice.

MSc or equivalent.

Minimum of 5 years post registration experience.

Minimum of 5 years recent primary and community nursing experience.

Mentor/teaching qualification.

Clinical supervision training and experience.

Extended/supplementary prescriber.

Experience of developing and maintaining positive and effective partnerships with partner organisations.

**Knowledge**:
Understanding of the local healthcare system, its priorities and challenges.
