An exciting opportunity is available to join the Mental Health Social Care Team based at Hartley Hospital in Southport as a Community Care Practitioner. The role is part time, 18 hours per week.

As a CCP you would be expected to:

- Undertake needs-led assessments with the full involvement of service users, their carers and representatives.
- Negotiate appropriate care arrangements with service user and carers including access to the in-house Mental Health Recovery Team
- Monitor and review the provision of in-house services, placements and packages of care.
- Support service users to fully utilise in-house and voluntary sector services to maximise their well-being.
