An opportunity has arisen to join our team as a patient care coordinator.

We are looking for someone who will provide excellent customer service and is able to work in a busy and high pressure environment. This role is ideal for someone who is highly organised and an effective communicator who can provide a professional and warm welcome to our patients, ensuring every contact is of the highest standard. IT skills are essential with excellent attention to detail.

Your regular duties in this role will be dealing with telephone, face to face and electronic enquiries, booking and/or amending patient appointments and home visits, contacting patients to provide information from the clinical team and assisting patients to access our services and those available in the wider community. You will also process prescription requests and provide a full administrative service.

Working hours on a 2 week cycle shift pattern.

Monday to Friday

Morning shift 8am—1pm

Afternoon shift 1pm—6.30pm alternating weeks.

You will also be required to cover some enhanced access hours on a monthly basis. These are paid at an enhanced rate.

Currently we work one evening per week 6.30pm until 8pm and Saturdays 9am—12noon.

We offer a good rate of pay, NHS pension scheme and generous holidays.
