The first contact physiotherapist will, as part of a multi-disciplinary team, be in a patient facing role to clinically assess, diagnose and triage patients using your expert knowledge and will work alongside the general practice teams and our local Transformation Team. In addition you will be expected to take responsibility for the management of patients, providing care which is tailored to individual needs, including appraising the impact of general health and well being on their condition. To use specialist knowledge to refer to other clinicians as appropriate, eg orthopaedics and podiatry To establish a rapid and accurate diagnosis and management plan for patients presenting to primary care with an acute MSK condition, performing advanced assessment procedures To request and progress investigations to facilitate diagnosis and most appropriate treatment/intervention, whilst understanding the information limitations derived from these and the relative sensitivity and specificity of particular diagnostic tests including X-Rays, USS and EMG, and interpret and action results to aid diagnosis and management plans for patients Deliver programmes of supported self management for patients, encouraging behavioural change, physical activity and mobility, minimising the need for pharmacological interventions wherever possible To be accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence-based To be accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence-based practice. To be part of a multi-skilled primary care team providing patient centred medical services based on patient needs with a focus on continuity of care. Including;
- working as part of a multi-disciplinary primary healthcare team in a patient facing role taking responsibility for the care management of patients, communicating effectively with patients and their carers, including individuals with particular psychosocial and mental health needs and with colleagues across the primary care team, sectors and settings providing specialist expertise in the diagnosis and management of patients who require physiotherapy intervention, whilst understanding the process for effective resource utilisation demonstrate ability to identify and resolve/and or escalate risk according to PCN policies/protocols to be professionally and legally accountable for all aspects of your own work and the care of patients on your caseload to keep up to date with clinical developments, analysing current research and good practice and implementing changes accordingly developing relationships and work closely with other professionals within and across PCNs and the wider health. To be able to plan ,monitor and manage acute conditions/symptoms with an emphasis on single contact where possible To exercise a critical understanding of personal scope of practice, referring on when and where appropriate, whilst recognising where there are opportunities for developing the scope and competence of the first contact physio service and wider MDT to meet patient needs Decide on the best care pathway through patient assessment and in partnership with carers, informed by the urgency and severity of need, patient acuity and dependency, and the most appropriate deployment of resources To make use of digital technology with patient care where possible To communicate sensitive and complex information to patients, carers and other professionals in order to gain cooperation for the best possible treatment plan for patients presenting with complex and/or multi-pathological issues To utilise engagement, motivation and reassurance skills to maximise the cooperation of the patient in order to ensure the successful outcome of each treatment/management plan To share current research developments, evidence based practice and best practice guidelines To ensure good record keeping when sharing discharge information to registered GP and ensure that referral information to secondary care is accurate and timely To contribute to the PHCT, undertaking evaluation and audit activity to drive improvements Act as a source of MSK clinical expertise to GPs within the Network
