Days/Times**:Monday - Friday - 09:00am-17:00pm**
Job Description/ Duties: (DBS is required)**

Looking for a TA with experience of working with older people in acute, community or care home settings. You will build rapport quickly, are will be enthusiastic about rehabilitation and enjoy motivating older people.

**Main Duties of the Job**

The postholder will undertake delegated duties from a registered clinician (physiotherapist), they will have excellent communication skills, working closely with NWAFT colleagues and the care home to ensure continuity of rehabilitation and optimisation ofpatients’ function in preparation for discharge. The postholder will ideally have experience of acute and community settings and will be able to manage a caseload of patients confidently within the interim care home setting.
