Patient Navigator Cancer Services

Track & chase diagnostic investigations for all patients on specific cancer pathways ensuring these are booked to meet timed pathway requirements. Ensure results are available for Outpatient appointments and MDTs. Proactively liaise with patients on cancer pathway to ensure they are aware of diagnostic pathway expediting investigations whilst meeting their preferences reducing risk of wasted appointments. Process and track onward referrals from CNS team to support services to ensure completion.
Liaise with relevant staff to book OPAs in oncology and post-MDT follow up appointments for certain tumour sites Work with MDT coordinator to Track & chase surgical/tertiary hospital investigations including PET scans, keep patient informed. Support 62 day, upgrade and screening pathways, liaising with MDT coordinator Coordinate next steps in the pathway with patient Attend cancer PTL (Patient Tracking List) meetings for relevant tumour sites, action administration tasks. To carry out administrative and clerical duties as requested within the department in an efficient and confidential manner, for example photocopying, posting information, printing clinic lists. To communicate with patients families and carers in a sensitive and confidential manner working closely with clinical teams as required.
