Our Health Partnership are seeking Advanced Clinical Practitioner Paramedics who will be able to transition their ability to deliver high quality pre-hospital care into the general practice environment. The role will involve conducting telephone assessments, face to face consultations and home visits across practices within the Alliance of Sutton Practices Primary Care Network (ASP PCN).

You will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT) consisting of Pharmacists, Physician Associates, Physiotherapists, Social Prescribers, Health & Well Being Coaches and will benefit from joining an existing team of experienced paramedics who can support your development. Supervision from a lead GP, networking opportunities with other paramedics across various PCNs and opportunities to engage in CPD via the training hub will also contribute to enhancing your knowledge and skills.

**Main duties of the job**

The role will involve working as an autonomous, accountable Paramedic in a general practice setting. You will be required to assess, diagnose and treat a variety of presentations incorporating same day emergencies, minor illness/injury and management of long term conditions. We are looking for individuals who will be able to utilise advanced clinical assessment skills, provide a holistic approach to patient care and empower them to make informed choices about their treatment.

**Job responsibilities**

The role will provide a specialist Paramedic resource for Health Care Professionals and service users, working in collaboration with other members of the Primary Care Network (PCN) Multidisciplinary Team (MDT).

The post holder will be required to work in all GP Practices within the PCN and be responsible for assessing, diagnosing and treating undiagnosed and undifferentiated conditions across the age ranges. Patient assessment will be conducted via phone consultations, face to face appointments and home visits. The post holder should be able to recognise the need for referral or signposting of service users to alternative care pathways where appropriate. There will be a need to engage with a palliative care caseload, which would involve the assessment and treatment of acute and chronic presentations and engaging patients and their relatives in discussions about advanced care planning.

There is also the opportunity for the post holder to perform other clinical and administrative duties where trained and determined safe to do so by the organisation. They may also be required to assist with medical reports, safeguarding, attending clinical meetings and clinic support.

**Person Specification**

**Experience**

**Essential**
- Ability to conduct a systematic and comprehensive patient assessment of all systems and use this to work towards formulating a clinical diagnosis and treatment plan
- Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
- Working knowledge of the NHS, principles and values
- Ability to write comprehensive clinical notes, implement and evaluate care plans

**Desirable**
- Mentorship or supervisory skills training
- Awareness of data protection (GDPR) and confidentiality issues particularly within a PCN
- Minor illness management
- Telephone triage
- Long term conditions management

**Qualifications**

**Essential**
- BSc in a training programme approved by the College of Paramedics
- Health & Care Professions Council (HCPC) registration
- Advanced clinical practitioner status
- 5 years post registration
- Able to operate at an advanced level of clinical practice
- Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11
- Driving License and access to a vehicle

**Desirable**
- Membership of the College of Paramedics
- BLS trainer
