Our Integrated Frailty Service has an exciting opportunity for a Frailty Advanced Clinical Practitioner - Band 8a

Successful applicants will be crucial to our Integrated Frailty Service and play a pivotal role in developing services around Same Day Emergency Care (SDEC), Rapid Access clinics, Frailty short stay and our Frailty Virtual wards

**Location**: Grantham Hospital, but may be expected to be able to work across all sites within ULHT to meet the needs of the Frailty Service. There will also be a requirement to work as part of the vision for an Integrated Frailty Workforce for Lincolnshire and be part of the Frailty Virtual Ward.

- The service will run 7 days a week with hours eventually extending from 08:00 - 20:00

This is a fantastic opportunity to join our newly formed team and be a part of developing a truly Integrated Frailty Service.

Belton Frailty Assessment Unit (FAU) will be a multi purpose ward area ; home to Frailty SDEC patients comprising of 8 chairs operating daytime hours, 8 Frailty SDEC assessment beds and 8 low acuity inpatient medical beds. It will also be home to our newly developed Frailty Hub supporting patients through the Frailty Virtual ward to allow hospital level support and monitoring in the patients own home.

The Frailty ACP will use specialist knowledge and skills to provide autonomous assessment to patients.

The ACP will be accountable for independent clinical assessment, diagnosis and treatment of patients who are frail as part of the Integrated Frailty Service.

Be a key member of an truly Integrated Multi-Disciplinary Team

The aim is to provide prompt specialist assessments, to avoid hospital admissions where appropriate, to deliver a Same Day Emergency Frailty models of practice and to promote care closer to home through our Frailty Virtual Ward.
