High Peak CVS are delighted to be expanding their Social Prescribing Team with several new roles.
We are excited to be expanding our Social Prescribing team and have a number of vacancies available across the High Peak.
Our Social Prescribing team work closely with clients taking a holistic approach to people’s health and wellbeing. The available roles will be an important part of the team ensuring we help those who are referred to our service.
The current roles available are:
- Children & Young People
- Hospital Discharge
- Adult
- On Duty Triage
