0-19 Healthy Child Practitioner

Exciting opportunities have arisen for multiple new posts within the 0-19 Universal Service. Healthy Child Practitioners, along with other members of the 0-19 team, work with guidance from Specialist Community Public Health Nurses (SCPHN) to support children, young people and parents, delivering interventions and early help to ensure all children have the best chance of reaching their optimum potential.
**The post holder is expected undertake the following**:
Short term interventions with families to enable positive lifestyle choices and intervene early with issues such as sleep, eating/feeding, emotional wellbeing, toileting/continence
Parenting support
Care navigation/coordination when open to multiple services
Monitoring and support while waiting for other services
Working with other professionals to provide early help
Supporting the delivery of Healthy Child Programme
NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
