We are looking for proactive and dynamic Clinical Support Workers to join our multi-disciplinary team in our Community Integrated Response Team. The Home First team will focus on providing high quality, integrated care for patients from within the local adult population with sub-acute health care needs. Your role will be to provide person centred care to the patients in line with their care plan. The care will be planned and assessed by the Registered Nurses and Therapists and provided under the direct supervision of the registered nurses or other members of the multidisciplinary team.
**To join our multidisciplinary team you must have**:
- NVQ Level 2 in Health and Social Care, or demonstrable equivalent experience.
- caring and compassionate attitude
- a strong desire to help people
- good communication skills
The post involves a 7day working rota.
The directorate of Wirral Systems 1 covers a variety of teams. The Home First service supports people to return to their home after a stay in hospital and enables them to continue their recovery in their own surroundings.
Home First sits under the umbrella of the Community Integrated Response Team (CIRT)
Combining the skills of both Health and Social care in achieving clinical outcomes with integrated delivery of care being the prime objective of the directorate, with the adoption of innovative ways of working to ensure objectives are met Working collaboratively with individuals to develop and achieve their goals; optimizing independence and wellbeing in their own homes
