Our very busy Integrated Discharge Team is based at Whiston Hospital and is recruiting for a full time Assessment and Review Worker post. This post will include working bank holidays and weekends on a rota basis.
The team has mix of staff who work alongside the multi disciplinary team on each ward. The purpose of the post is to assess patients’ needs, arrange appropriate packages of care, facilitate safe and timely discharges, refer onto voluntary organisations, identify carers’ needs and signpost to the most appropriate service, for information and support. We will support you with professional supervision, appraisals and training. The team is part of Contact Cares and benefits from also working with services in the community.
