Older People and Stroke Service in RAH is offering an exciting opportunity to be part of a multidisciplinary team with responsibility for assessment of care needs and the development of programmes of care, and / or the implementation and evaluation of these programmes ensuring the delivery of high quality care to patients. The role will include assessment of presenting conditions, assessment of falls, fracture and frailty risk factors, initiation of investigations and appropriate treatments, triage and facilitation of onwards transfer of patients to rehabilitation wards, liaison with the Fracture Liaison Service, next of kin, community falls and rehabilitation teams, and where appropriate discharge of patients with arrangement of relevant follow up.
