- Drivers preferred. As a Support Practitioner you will support people with all aspects of daily living at home and in their communities. You will assist people with planning for their future and to prevent the effects of social isolation and loneliness in their lives. We adopt a person centred approach to working with individuals who require support for a variety of reasons including Mental Health, Autism, Physical & Learning Disabilities.
