**Lincoln Healthcare Group Ltd are looking for 2 Evening and Weekend On-Call Care Coordinators who will be working from home to join our friendly team.**

**Positions Available** Evening and Weekend On-Call Care-Coordinators required on a rolling rota pattern as follows

**Week one**:

- Monday, Tuesday, Wednesday & Thursday Evenings - 4pm - 9.30am (sleeping at home payment for hours 11pm-7am) = 28 hours plus sleeping hourly pay.

**Week two**:

- Friday 4pm - Monday 9.30am (sleeping at home payment for hours 11pm-7am) = 41.5 hours plus sleeping hourly pay.

**Job requirements**

You must have the ability to work independently, maintain confidentiality of information, meet deadlines and effectively multi-task.

Provide out of hours telephone assistance for all issues arising from our clients or support workers.

Arranging alternative cover to provide care if someone becomes unavailable (e.g., due to staff illness)

Liaise with the other co-ordinators for weekly hand overs of new clients and support workers.

Update any rota changes on Carefree. (Training will be given on Rota software).

Manage call monitoring technology as per company contractual agreements.

Give direction to support workers in line with policies and procedures when issues arise and be able to deal with emergencies.

Undertake any other actions that may be required during out of hours to support the business and maintain quality for clients.

Liaise with Adult Social Care during out of hours for any issues relating to local authority care packages.

Liaise with family and other care professionals as required to resolve customer issues.

Maintain GDPR guidelines when working remotely

Adhere to safeguarding protocols in relation to all clients, staff, and external stakeholders.

Be IT literate or be willing to attend training. You will ideally have access to Internet / WiFi at your home address or be wiling to have this provided at a cost to the employer. Having a private space to carry out your role is required to meet GDPR guidelines.
