An exciting opportunity has arisen for an experienced healthcare professional to join the North Locality Integrated Care Service (LINCS). LINCS is a responsive and coordinated multidisciplinary care approach for patients over (18+) living in Perth and Kinross, who are assessed as being medically safe to be cared for at home. The support is intended to be a timely and short escalation of care during which time a person’s needs are managed by the Clinical Coordinator in conjunction with the key clinicians within the team. The main aims are to improve patient experience and outcomes whilst reducing the need for unnecessary emergency admission or readmission to hospital or long term care.

The post holder will be responsible for the clinical triage of all referrals to LINCS, ensuring an appropriate care pathway is established for each individual. This requires advanced clinical knowledge and skills to holistically assess those with complex care needs, excellent communication and leadership skills, and the ability to work confidently and proactively as part of an integrated multidisciplinary care delivery team.

We are looking for an individual who has experience of clinical assessment and caring for people with complex care needs. You will have advanced verbal and non-verbal communication skills. The ability to work well under pressure and respond positively to unpredictable situations is essential.

We are committed to ensuring staff feel supported with their wellbeing and development through supervision, appraisal and training. Staff are expected to be part of a variety of Quality Improvement initiatives.

You must be a First Level UK Registered General Nurse or AHP equivalent with significant post registration experience, with current NMC or HCPC registration.

**Hours of work are**: 37.5 hours (location base can be discussed at interview, flexible between Blairgowire or Pitlochry)
