We are looking for a **Triage Officer **will be based at:
Duties and Responsibilities - Job Specific
- To be the lead officer from within the CFW team with responsibility for promoting the project and its objectives and engaging with those hard to reach participants in the Communities First areas and other agencies operating locally.
- To be the initial point of contact for all self-referrals and agency referrals to the CFW delivery team. This will include within the CFW office and also at other access points within the Communities First cluster.
- To be responsible for undertaking an initial participant assessment and based upon this assessment allocate or refer participants to appropriate eligible services or projects.
- To support the local CfW delivery team in the development, implementation and ongoing operation of CfW within the cluster.
- To ensure that all administrative processes relating to monitoring, updating of participant records, performance management information, reports and other monitoring requirements comply with all Welsh Government and ESF monitoring. To ensure that all submissions meet deadlines and are accurate.
- To be responsible for the administration of the local delivery team and support the maintenance of records in accordance with ESF audit requirements and Conwy County Borough Council’s audit requirements.
- To lead on the Triage process within the Cluster and ensure that all relevant information is provided on each participant in advance of each referral meeting ensuring that each record is moderated.
- To be familiar with all other projects and initiatives that support unemployed adults and young people in the cluster area
- To ensure that all personal data is effectively protected and handled, within the locally agreed Information Sharing Protocols.
- To work as an integrated team with the Youth and Adult Mentors, DWP Adult and Youth Advisors and the Conwy Communities First Team.
- To have responsibility for administering petty cash arrangements including reconciliation.
- To participate in the Welsh Government CfW Programme development and training activities.
- To carry out other tasks and responsibilities of a similar nature as determined from time to time by the line manager in relation to the smooth running of the service
Duties and Responsibilities - Corporate
- To be responsible for establishing good working relationships both internally and externally.
- Compliance with the Authority’s Policies and Procedures and to make known to Senior Officers any areas which are not adequately covered.
- To participate actively in supporting the Authority’s principles and practices of equality of opportunity as laid down within the Equalities Policy.
- Employees will be required to give certain information relating to themselves in order that the Authority may properly carry out its duties, rights and obligations as an employer. The Authority will process and control such data principally for personnel, administrative and payroll purposes.
- As a term of your employment and in order to maintain effective departmental operations, you may be required to undertake any other reasonable task, commensurate with your grade, as determined by your Line Manager or Head of Service.
