We are in search of dedicated Support Workers who are passionate about empowering individuals to achieve their dreams while respecting their cultural preferences and personal aspirations.

Key Responsibilities:

- Exemplary Care: Delivering exceptional support that aligns with personalized support plans, cultural needs, and individual desires.
- Home Management: Assisting individuals in effectively managing their homes, ensuring successful tenancy upkeep, and providing guidance during transitions.
- Personal Care: Offering dignified assistance with personal care activities, such as toileting and bathing, in accordance with support plans.
- Empowerment and Decision-Making: Encouraging and supporting individuals in making choices and life decisions, adhering to the principles of the Mental Capacity Act.
- Engagement: Inspiring participation in daily home, social, leisure, and work activities, respecting individual preferences.
- Health Support: Helping with health-related needs, including medication administration, and accompanying individuals to medical appointments.
- Accurate Records: Maintaining meticulous and up-to-date records to ensure transparent communication and accountability.
- Financial Guidance: Offering necessary support in financial matters, in alignment with Hft guidelines, and promoting financial independence.
- Well-being Promotion: Championing the health, safety, and overall well-being of the individuals under our care.
- Collaborative Approach: Building positive relationships with colleagues, families, external organizations, and professionals.
- Innovative Initiatives: Identifying opportunities to leverage Personalized Technology to enhance individuals' independence and quality of life.
- Confidentiality: Upholding a high standard of confidentiality to ensure individuals' privacy is safeguarded.

**Requirements**:

- Previous experience in care homes, supported living, or community care settings.
- Genuine passion for supporting individuals with learning disabilities.
- Excellent communication skills, empathy, and interpersonal abilities.
- Flexibility to work varying shifts to accommodate individual needs.
- Strong commitment to maintaining confidentiality and respecting privacy.
- Familiarity with the Mental Capacity Act and its principles.
